I hereby grant permission to Grove Christian Outreach Center to photograph, record, and/or capture my image and likeness through any audio, visual, or digital media. I authorize Grove Christian Outreach Center, its representatives, assigns, and transferees to use, reproduce, copyright, and publish such media in print and/or electronic formats.

I understand that these materials may be used with or without my name for any lawful purpose, including but not limited to publicity, promotional materials, educational activities, special events, illustrations, and web content, as deemed appropriate by Grove Christian Outreach Center.

Additionally, for organizations and groups volunteering with or participating in activities that benefit Grove Christian Outreach Center, this consent extends to their use of such media to promote or document their service in connection with the Center.

By signing below, I acknowledge that I have read, understood, and agree to the terms outlined above.

I agree to maintain the confidentiality of all information to which I may have access regarding clients of Grove Christian Outreach Center. I will not remove any electronic or written records—or copies thereof—from the premises without the express permission of Grove Christian Outreach Center or the client involved.

I accept full responsibility for safeguarding all confidential information, whether verbal or written, and understand that I am personally accountable and fully liable for any breach of this agreement.
